Hy-Vee's sweepstakes is open to Kansas residents age 18 or older through September 28, 2026. According to the Hy-Vee Customer Experience Survey Sweepstakes rules, one $500 gift card is awarded in each of 12 survey periods, totaling $6,000 in prizes. A Wichita-area entrant can bypass both a purchase and the customer survey.
The mail-in method requires a handwritten 3-by-5-inch card containing the specified contact information and birth-date details. Entries are limited to one per person or email address per day.
